Concierge Medicine – Is it for you?

The last time I called the doctor’s office, how long you must wait for an answer? How long do you usually have to wait for an appointment or wait for a prescription to be called? Arriving at the office, how much time you spend in the waiting room and how long the doctor spends with you?

Did you get all the answers to your questions? How many seconds do you have in the beginning of his appointment to explain your symptoms before they stop? Do the nurses and doctors seem to be more interested in their computers or their cards than they are for you?

If your experience is like most, the answers to these questions are not very flattering to the medical profession and the health system in general. Most doctors do not really want to have their practices as well, but do not have much choice. They have to have a high volume of patients in order to gain economic life. The high volume makes a very busy clinic and most patients do not feel they receive much attention.

In 1996, in Seattle, a doctor named Howard Moran thought it should be a better way to do this. He pioneered the concept of having a lower volume practice with attentive care provided as a service to patients in exchange for a retainer fee, as many lawyers or accountants use. This fee may be in addition to, or instead of, office fees charged to insurance companies. This concept allows the practice to remain financially solvent and provide better medical service more attentive to their low volume of patients (usually the patient keep the countdown for about a tenth the number in a typical traditional practice of primary care ).

Unfortunately, health insurance companies currently do not pay for this service, which means that patients have to pay out of pocket, but if the service is good, it may be worthwhile. Patients who join these practices are encouraged to maintain regular insurance is required for visits to other specialists, laboratory tests, radiology tests and / or hospital services if necessary.

Many concierge practices offer appointments the same day or the next, without having to wait for appointments or fill prescriptions, direct access to your personal physician day or night, home visits if necessary, continue care in case of hospitalization, complete physical examination, audiometric tests, cardiovascular and cancer risk detection, greater attention to preventive care, unhurried appointments, all questions answered, family reunions, if necessary, coordination of care with specialists, the provision of personal health records on CDs or flash drives, etc, etc

There has been some controversy surrounding some of these practices because some authorities feel that concierge medicine growth will result in a 2-tier medical system in this country – one for the rich and another for the not so rich. And with the current shortage of primary care physicians (PCP), with a growing number of concierge practices, the number of primary care physicians offices will be even less traditional, and access to care for patients served by these practices will be affected. On the other hand, more medical students may become interested in primary care if the opportunities in concierge medicine are available for when they finish their residencies.
